Consent & Confidentiality Statement

As a Registered Social Worker (RSW) with the Ontario College of Social Workers and Social Service Workers (OCSWSSW), I am held to strict ethical and professional standards. These standards require me to maintain confidentiality, act with integrity, and prioritize your well-being throughout our work together. I also have a professional duty to act in your best interest and to inform you of any limitations in confidentiality, as outlined below. 

By submitting an inquiry or engaging in communication with Cognitive Alignment, whether by phone, email, contact form, or in the course of participating in therapy, you acknowledge that your information may be collected for the purpose of providing services or responding to your request. Participation in therapy is entirely voluntary, and you have the right to withdraw your consent or discontinue services at any time. 

Your privacy is important to us. All personal information you share, whether during inquiry, consultation, or in the context of therapy, is treated as confidential and is used solely for service delivery and communication. This confidentiality is upheld during and after the professional relationship has ended. 

However, there are specific legal and ethical exceptions to confidentiality, including the following:

• If I believe that a child is, or may be, at risk of abuse or neglect, or in need of protection;

• If I believe that you or another person is at clear risk of imminent harm;

• If I am required to disclose information due to legal proceedings or if maintaining confidentiality would contravene the law.
